COVID-19 VACCINATIONS

Please select your state of residence to see the vaccination services and locations for you or your patient.
In-Home Vaccinations
To book an in-home COVID-19 vaccination you must be a Rhode Island resident. Please fill out the following form for you, a family member, or patient and we will contact you back to book the appointment.
* At this time we are offering seasonal Flu as well but can only be provided if a COVID-19 vaccination is booked at the same time.
In-Facility Vaccination Clinics
Nursing Facilities, Assisted Living Facilities, Group Homes, etc., in the state of Rhode Island can book all of their COVID-19 vaccinations (including booster clinics) by contacting us at AlertMIHC@AlertEMS.com, please make sure you include the number of residents/staff and what brand they will be receiving.
After confirming a date we will create a private PrepMod clinic for your facility and send you the link to pre-register all patients.

Mobile Integrated Healthcare (MIHC)
What is MIHC?
Mobile Integrated Healthcare is a fairly newer healthcare model where progressive healthcare companies like Alert Healthcare bring wellness directly to the patients home. We partner with hospitals and ACO’s to identify high risk patients with conditions such as COPD, CHF, Diabetes, etc., and provide them with preventative care in the safety of their own home. This preventative care has been proven to decrease hospital re-admissions in high risk patients.
Most high risk patients do not get the time and attention they need to keep up with medication changes and have all of their medical questions answered. We will send a trained clinician to the patients home for a vital sign check up and can perform the following at each visit:

Blood Pressure, Heart Rate, Lung Sounds, SpO2 (pulse oximetry), ECG (Electrocardiograms), 12-Lead ECG’s, Blood Sugar Analysis, ETCO2 (End Tidal Carbon Dioxide), Weight and more.
